Torbay and Southern Devon Health and Care NHS Trust has been selected by Ostrich to provide a 24 hour a day response as part of their GPS Tracking Service.
The Trust provides community health services to around 375,000 people in Torbay and Southern Devon, and in Torbay it provides and commissions adult social care services.
Its 2000 staff include frontline health and social care staff, such as district nurses, occupational therapists and social workers, and operate from a range of different premises across Torbay and south Devon, such as community hospitals and clinics.
The Trust’s aim is to provide ‘the right care, in the right place, at the right time’. It seeks constantly to:
Within Torbay, staff at the centre work very closely with organisations that provide specific support to individuals and these include:
In 2013 the centre won a grant from the National Dementia Challenge Initiative to develop a GPS tracking service within Torbay that supports the local population. This project also involves measuring the effects that the GPS service has on both those that suffer from Dementia and their Carers.
As an NHS organisation, the Trust has a duty to monitor the performance of its own services and the services it commissions to ensure they are safe, high-quality, and efficient.
Monitoring and measuring the performance of services also enables the identification of where improvements can be made to further enhance service quality.
There are a number of different performance measures, including those provided by external watchdogs, such as The Care Quality Commission, a regulatory body which benchmarks how well all Trusts are doing.
The Company Secretary produces a regular report which provides an update of organisational assessments.
The Annual Reports produced by the various directorates within Torbay and Southern Devon Health and Care Trust also provide information on its performance.
Our GPS Tracking Service is designed to give peace of mind to both the individuals suffering from dementia (Alzheimer’s, Lewy Body, Pick’s disease…) and their carers.
